There is nothing more important than Root Cause Analysis. It is the “A” in DMAIC (if you are a fan of Six Sigma), and yet so few people truly understand it! It is the basic foundation of all of the Lean tools. In other words, 5S, Total Productive Maintenance, Quick Changeover, etc., are the preventive solutions to one company’s complete root cause analysis on a problem that actually occurred. What is important is not the solution, or the tool, but the root cause analysis that was performed. It is the foundation of ISO 9001 and its attempt at getting your company to “continually improve the effectiveness of the Quality Management System”.
